Important Information regarding Orthotics

 

Congratulations on taking your first step towards better foot health!

 

Here are the procedures to the orthotic process at Kim Family Chiropractic.

 

1. ASSESSMENT

a. A Chiropodist (foot specialist) will gather health history information.

b. A biomechanical evaluation will be performed to determine if custom-made orthotics/shoes are necessary.  If orthotics/shoes are prescribed, you may then choose to have a cast taken of your feet.

c. A prescription for your insurance is provided.

 

2. CASTING/SELECTION

a.  A foam casting kit is used to make an exact impression of your feet.

b. Impressions will be sent to the orthotic lab to create your custom orthotics.

c. Select your choice of orthotics or custom-made orthotic shoes (please make two selections of shoes in case your first choice is unavailable).

d. Order should be ready in 10 - 20 business days.

 

3. PAYMENT/FEES

a. Orthotics or orthotic shoes are covered at 100% once a year, if your Sunlife contract number is 025168.  If your contract number differs from 025168, then coverage may vary and is dependent on your policy.

b. The cost for custom-made orthotics is $500.  Orthotic shoes, however, range from $550 to $850, depending on your shoe selection.

c. Insurance can be filed on your behalf once full payment has been made and you are completely satisfied.  We require your Member ID number and policy holder’s signature.

d. OR …We will mail an insurance package to you within two to three days after full payment has been received.

e. We accept cash, personal cheque, Visa, MasterCard and AMEX payments.

 

4. WARRANTY

a. Exchanges for size and style are permitted within 3 months of purchase provided the footwear has not been discontinued and does not show any signs of wear.  If there are any concerns at all regarding your order or procedures, please do not hesitate to call or email us.  Your satisfaction is our commitment.
